By signing this form, I give consent for my child/children to participate in the children's ministry of Emmanuel Church. I understand the nature of the activities involved and grant permission for my child to attend and participate. I also authorize any necessary emergency medical treatment for my child during their participation. I accept that Emmanuel Church will be taking pictures throughout the event and that my child's picture may appear on their website/social media.
